Job Description:

Sanford Health, a leading health system in the United States, is seeking a compassionate and dedicated Social Worker to join our Social Services department in Hillsboro. This role involves providing critical social support services to patients and their families, helping them navigate the challenges associated with their healthcare journey. As a Social Worker at Sanford Health, you will work closely with a multidisciplinary team to ensure comprehensive patient care and support across our clinics, hospitals, and senior living facilities. Assess patient needs and develop appropriate intervention plans. Coordinate with healthcare providers to ensure patient access to necessary medical and social services. Advocate for patients and their families to facilitate access to resources both within and outside the healthcare system. Conduct regular follow-ups with patients to assess their progress and update intervention plans as needed. Provide crisis intervention services as required. Maintain comprehensive documentation of patient interactions and interventions. Strong interpersonal and communication skills. Ability to navigate complex healthcare systems. Crisis intervention expertise. Proficiency in case management software and patient databases. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Licensed Clinical Social Worker (LCSW) preferred. Certified Social Work Case Manager (C-SWCM) is a plus.
