Job Description:
Good Samaritan Health Center seeks a dedicated RN, Minimum Data
Set (MDS) Coordinator, who aligns with our mission to provide
holistic healthcare to underserved communities. The candidate will
oversee the comprehensive assessment of clinical data of our
residents, ensuring compliance with federal and state regulations,
optimizing health outcomes and improving the quality of life for
our patients. Coordinate and complete the RAI process ensuring
accuracy and timeliness. Analyze patient assessments and care plans
to guarantee comprehensive care. Collaborate with healthcare
providers to tailor interventions based on MDS insights. Maintain
up-to-date knowledge of all regulations regarding nursing care in
long-term settings. Educate staff about documentation guidelines
within the MDS framework. Proficiency in RAI process and MDS 3.0.
Strong analytical skills and attention to detail. Excellent
communication and interpersonal skills. Ability to lead and educate
nursing staff. Experience with electronic health record (EHR)
systems. Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Active RN license in the state of practice.
Certification in MDS Coordinator preferred (e.g., CAA or
RAC-CT).
