We are looking for a committed RN Case Manager to join our team and contribute to our mission of offering compassionate, comprehensive patient care. In this role, you will be responsible for assessing, planning, facilitating, and advocating for options and services through a continuum of care from point of contact through discharge on assigned cases. This position will have a significant impact on the quality of care offered and the ongoing success of our healthcare initiatives. Conduct comprehensive patient assessments to determine their needs and goals. Develop, implement, and review individualized patient care plans. Facilitate coordination and continuity of care through efficient communication with all healthcare providers, patients, and their families. Monitor and evaluate patient progress and modify treatment plans as necessary. Ensure compliance with healthcare regulations and standards. Maintain detailed records of patient care, condition, progress, and concerns. Provide education and resources to patients and their families to improve their health and wellness outcomes. Strong clinical skills in a healthcare setting Excellent organizational and communication skills Ability to make critical decisions under pressure Proficient in healthcare management and case management software Deep understanding of patient health care plans and patient management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Registered Nurse (RN) license. Certified Case Manager (CCM) preferred.
