A company is looking for a Care Coordinator, Per Diem - Weight
Management.
Key Responsibilities
Collaborate with a multi-disciplinary team to ensure a high-quality
patient experience
Ensure adherence to processes for efficient and timely delivery of
care
Manage patient/provider messaging and coordinate treatment and
community resources
Required Qualifications, Training, and Education
One year of experience in a medical practice with knowledge of
medical terminology
Willingness to work regular weekends and at least three holidays a
year
Self-motivated with a service-oriented mindset
Skilled in using online tools and technology for communication
Reliable attendance and adherence to the assigned work schedule
